    Burnout y satisfacción laboral en trabajadores/as sociales de la Región de Ñuble, Chile

    El objetivo de este artículo es analizar el nivel de burnout (agotamiento) en trabajadores/as sociales de la Región de Ñuble de Chile y estudiar la asociación de fenómenos laborales adversos -altas demandas emocionales, acoso laboral, vulnerabilidad laboral y sobrecarga laboral- con burnout y entre burnout y dichos fenómenos con insatisfacción laboral. Se realizó un estudio transversal mediante la aplicación de un cuestionario de auto reporte a 173 trabajadores/as sociales (143 mujeres y 30 hombres) de la Red Intersectorial de Trabajadores/as Sociales de la Región de Ñuble. Los resultados muestran que el 6.9% de los encuestados presenta síndrome de Burnout. En los análisis bivariados se observa asociación de altas demandas emocionales, presencia de acoso laboral y sobrecarga laboral con Burnout. Además, se encontró una asociación de Burnout, acoso laboral los últimos 6 meses, vulnerabilidad e inestabilidad laboral con la insatisfacción laboral. Al ajustar por atención de público y años de trayectoria se encontró que los trabajadores con altas demandas emocionales (OR= 6,811), acoso laboral [OR=5,430) y sobrecarga laboral (OR=4,431) tienen mayor chance de presentar Burnout que aquellos/as que no reportan dichas condiciones. Por último, se realizó un análisis por tramo de sueldo y tipo de contrato, los que reportan burnout (OR= 4,042), vulnerabilidad laboral (OR=2,464) o inestabilidad laboral (OR=2,526), tienen más chance de presentar insatisfacción laboral que los que no reportan esos fenómenos.

    Recommendations to strengthen hybrid mental health policy in primary care services in Chile

    <jats:title>Abstract</jats:title><jats:p>This article presents the main results of a qualitative study proposing guidelines for a hybrid public policy on mental health at the primary care level that faces the challenge of improving access under conditions of quality. The empirical evidence was constructed from semistructured interviews with users, decision makers, and professionals from Chile's central, regional, and local healthcare levels and a review of specialized literature. The study shows that strengthening a hybrid telehealth policy at the primary care level is an outstanding strategy to contribute to reduce accessibility, acceptability, and availability barriers. Yet, to be effective, it is necessary to take on some guidelines in terms of remote care management in Primary Healthcare centers, training of professionals, and skill building of civil servants in the use of technology, regulatory framework, infrastructure and technology; financing; inclusion of the user's perspective; and clinical guidelines and technical standards for hybrid care. The leadership of the Ministry of Health is fundamental to promote relevant regulation and the coordination and active participation of different stakeholders.</jats:p>

    Factors associated with loneliness in Latin-American family care partners during the COVID-19 pandemic

    <jats:sec><jats:title>Background</jats:title><jats:p>COVID-19-related restrictions led to an increase in overall loneliness and social isolation. Before the pandemic, care partners reported higher levels of loneliness and higher loneliness prevalence compared to non-care partners. Because of the spread and severity of the infections, and the access to support spread, we expect a different impact of the COVID-19 pandemic on LATAM care partners.</jats:p></jats:sec><jats:sec><jats:title>Objectives</jats:title><jats:p>To describe the loneliness levels of LATAM caregivers and to identify socioeconomic and health factors associated.</jats:p></jats:sec><jats:sec><jats:title>Design</jats:title><jats:p>An international online cross-sectional survey for care partners, embedded within the ‘Coping with Loneliness and Isolation during COVID-19’ (CLIC) Study conducted between June 2020– and November 2020.</jats:p></jats:sec><jats:sec><jats:title>Setting</jats:title><jats:p>We analysed data from 246 family care partners living in Latin American countries (46% Mexico, 26% Chile,18% Brazil, and 10% from Argentina, Peru, Venezuela, Panama, Guatemala y Costa Rica).</jats:p></jats:sec><jats:sec><jats:title>Measurements</jats:title><jats:p>We assessed loneliness using the 6-items of De Jong Gierveld loneliness Scale. We described the levels of overall, emotional, and social loneliness pre and during Covid, and reported the distribution of care partners who improved, worsened or maintained their levels of loneliness. Moreover, we used longitudinal multiple linear regression models with bootstraps errors of 1,000 iterations to identify factors associated with the levels of overall, emotional, and social loneliness during the pandemic.</jats:p></jats:sec><jats:sec><jats:title>Results</jats:title><jats:p>Participants were mostly women, 50 years and older, in a partnership, highly educated and with finances meeting their needs, with good to excellent physical and mental health. Among the total of care partners, 55% perceived higher overall loneliness, 56% higher emotional loneliness, and 21% higher social loneliness during the pandemic in comparison with pre-COVID-19 levels. Perceived mental health was associated with the overall, emotional, and social loneliness.</jats:p></jats:sec><jats:sec><jats:title>Conclusions</jats:title><jats:p>Regardless of their living and health situation, during the pandemic, loneliness increased in all groups of care partners. These should be taken in consideration when planning public health approaches for crises such as pandemics or other large-scale disruptive events.</jats:p></jats:sec>

    Síndrome de Ovario Poliquístico y Salud Mental en Adolescentes

    <jats:p>El Síndrome de Ovario Poliquístico (SOP) es el desorden endocrino-metabólico más frecuente en las adolescentes. Se asocia a complicaciones metabólicas, cardiovasculares y reproductivas. Hallazgos recientes sugieren también una asociación con patología psiquiátrica, tanto en las pacientes afectadas como en su descendencia. En la presente actualización, se realizó una síntesis de la literatura reciente relativa a la salud mental en mujeres y adolescentes con SOP. Se realizó una búsqueda sistematizada en PubMed, Epistemonikos y Scielo de los artículos publicados en los últimos 5 años. Se confirmó que existe un aumento significativo del riesgo de presentar trastornos por ansiedad, depresión, trastornos alimenticios, trastorno obsesivo compulsivo, trastorno afectivo bipolar y psicosis en mujeres con SOP. Además, presentan riesgo de trastornos del neurodesarrollo, tales como trastorno del espectro autista y trastorno por déficit atencional e hiperactividad, tanto en ellas como en su descendencia. La hiperinsulinemia y el hiperandrogenismo podrían explicar parte de estas asociaciones, afectando la maduración del sistema nervioso central, especialmente durante la vida intrauterina y la adolescencia. El distrés derivado de las características fenotípicas físicas de la enfermedad también podría impactar la salud mental de las pacientes. Aún no se cuenta con suficientes estudios que expliquen el origen de esta correlación, ni tampoco con ensayos clínicos que aborden la salud mental de forma específica en estas pacientes. La evidencia actual sugiere la necesidad de evaluar activamente la salud mental de estas pacientes y coordinar a los distintos equipos de salud que intervengan en el manejo de la patología.</jats:p>
