Item type:Publication, Test de Aprendizaje y Desarrollo Infantil (TADI): Evidencia adicional de su validez a nivel poblacional(Sociedad Chilena de Pediatria, 2025-02-18) ;Marta Edwards ;Iván Armijo; ;Marcela PardoAntonia ValdésEl “Test de Aprendizaje y Desarrollo Infantil” (TADI) es una escala de tamizaje del desarrollo para niños/as de 6 a 72 meses construida en Chile.Objetivo: Determinar la validez de la primera edición de TADI muestra nivel poblacional.Metodología:Análisis psicométrico secundario de una muestra de 11.283 niños/as chilenos que participaron de la Encuesta Longitudinal de Primera Infancia (ELPI) 2012. Se evaluó validez convergente mediante el Análisis Factorial Confirmatorio (AFC), verificado mediante Análisis Factorial Exploratorio (AFE). Se consideraron variables sociales para la validez de constructo. Para la validez concurrente, sensibilidad, especificidad y correlación se tomaron como referencias las pruebas Battelle Developmental Inventory Segunda Edición (BDI-2) y el Test de Vocabulario en Imágenes Peabody (TVIP).Resultados: De los niños evaluados, 12,6% se categorizó en Retraso o Riesgo de Retraso, y 17,3% en categoría Alerta. El AFC arrojó buenos niveles de ajuste del modelo propuesto para los datos de la escala, en particular a edades mayores. El AFE reveló patrones diferentes para distintos rangos etarios. El TADI se correlacionó significativamente con los factores socioeconómicos, como la presencia de materiales de aprendizaje en el hogar, el nivel educativo del cuidador principal, el ingreso económico y la respuesta emocional de la madre. Se encontró correlación moderadamente positiva del TADI con el BDI-2 y el TVIP, con sensibilidad y especificidad cercanos a 0,70 al incluir la categoría Alerta.Conclusiones: Los adecuados valores psicométricos del TADI en una muestra poblacional ratifican su validez como instrumento de tamizaje del desarrollo de niños/as pequeños en Chile.2 - Some of the metrics are blocked by yourconsent settings
Item type:Publication, Proposal of minimum elements for screening and diagnosis of gastric cancer by an international Delphi consensus(2022) ;Naomi Kakushima ;Mitsuhiro Fujishiro ;Shannon Melissa Chan ;George Adel CortasMario Dinis‐Ribeiro<jats:title>Abstract</jats:title><jats:p>The World Endoscopy Organization Stomach and Duodenal Diseases Committee extracted minimum elements for screening and diagnosis of gastric cancer (GC) in aim to support countries that do not have national guidelines on screening and diagnosis of GC. Current national or international guidelines were collected worldwide and recommendations were classified according to the quality of evidence and were finalized through a modified Delphi method. The minimum elements consist of seven categories: [1] Extraction of high‐risk patients of GC before esophagogastroduodenoscopy (EGD), [2] Patients who need surveillance of GC, [3] Method to ensure quality of EGD for detection of GC, [4] Individual GC risk assessment by EGD, [5] Extraction of high‐risk patients of GC after EGD [6] Qualitative or differential diagnosis of GC by EGD, and [7] Endoscopic assessment to choose the therapeutic strategy for GC. Item type:Publication, Detección temprana de Trastorno del Espectro Autista: revisión de las herramientas de tamizaje validadas en Chile(2023) ;Luisa Schonhaut B. ;Veronica Buron K ;Roxana Aguilera E.Lorena Vargas B.<jats:p>La pesquisa y derivación temprana del trastorno del espectro autista (TEA) son claves para potenciar el desarrollo de los niños afectados. Recientemente se promulgó una ley que indica realizar tamizaje de TEA en las prestaciones de salud. Esto representa un desafío para los profesionales de pediatría ambulatoria. El objetivo es revisar la utilidad en el tamizaje de TEA, de cuestionarios y escalas validadas en Chile para el screening del desarrollo psicomotor, desarrollo socio-emocional y cribado específico de TEA. El screening del desarrollo psicomotor evalúa el desempeño general desde edades tempranas, dando las primeras señales de retraso, con mayor probabilidad de que se trate de TEA si hay compromiso de los dominios social y lenguaje. Los cuestionarios socio-emocionales pueden ser aplicados precozmente, entre sus ítems se incorporaron preguntas de señales de TEA, pero no se usan rutinariamente en atención pediátrica. Las escalas específicas de screening de TEA validadas en Chile son Cuestionario Modificado de Detección del Autismo en Niños y Niñas Pequeños (M-CHAT), Quantitative Checklist for Autism in Toddlers Q-CHAT (Q-CHAT) y el Examen del Estado Mental para Autismo (AMSE), para lactantes mayores de 15/18 meses, con propiedades psicométricas prometedoras. Se recomienda complementar la observación clínica con pruebas generales para el tamizaje del desarrollo psicomotor, desarrollo socio-emocional y escalas específicas para el screening de TEA, en forma seriada en la infancia. La implementación de cuestionarios en línea permite optimiza su cobertura e interpretación. Independiente del dominio afectado, es importante realizar un seguimiento y comenzar con intervención temprana, mientras se realiza la confirmación diagnóstica. </jats:p>Scopus© Citations 3 2 - Some of the metrics are blocked by yourconsent settings

Item type:Publication, Acceptability of HPV Vaccines: A Qualitative Systematic Review and Meta-Summary(2023) ;María-Teresa Urrutia ;Alejandra-Ximena Araya ;Macarena Gajardo ;Macarena ChepoRomina Torres<jats:p>In 2006, the human papillomavirus (HPV) vaccine was approved for use as an effective intervention for reducing the risk of developing cervical cancer; however, its successful implementation is dependent on acceptability. This study aims to provide a comprehensive understanding of the reasons that favor or do not favor the acceptability of HPV vaccines. Methods: We conducted a systematic review and meta-summary of qualitative research on 16 databases. A total of 32 articles that considered the perspectives of vaccine users, their parents, and the professionals who care for them were reviewed. Synthesis was conducted as described by Sandelowski and Barroso. Results: We used inductive and deductive methods to obtain a total of 22 dimensions, out of which three issues stood out that should be considered to improve acceptability and are formed by three groups of study, namely, information about the vaccine, fears and side effects, and sexuality associated with the vaccine. Conclusions: Acceptability, as well as adherence to HPV vaccination, is a complex concept. This review highlights the perspectives of the three sets of actors involved in the process (i.e., users, parents, and professionals) and views these factors in relation to acceptability as a guide for new interventions.</jats:p>6Scopus© Citations 7 - Some of the metrics are blocked by yourconsent settings

Item type:Publication, Modelling cost-effectiveness of syphilis detection strategies in prisoners: exploratory exercise in a Chilean male prison(2021); ;Pedro Gajardo ;Manuel Nájera-De Ferrari; Macarena Hirmas-Adauy<jats:title>Abstract</jats:title><jats:sec> <jats:title>Background</jats:title> <jats:p>Syphilis, together with other sexually transmitted infections, remains a global public health problem that is far from controlled. People deprived of liberty are a vulnerable population. Control activities in prisons rely mostly on passive case detection, despite the existence of affordable alternatives that would allow switching to active case-finding strategies. Our objective was to develop a mathematical modelling framework for cost-effectiveness evaluation, from a health system perspective, of different approaches using rapid tests for the detection of syphilis in inmates' populations and to explore the results based on a Chilean male prison population.</jats:p> </jats:sec><jats:sec> <jats:title>Methods</jats:title> <jats:p>A compartmental model was developed to characterize the transmission dynamics of syphilis inside a prison with the ongoing strategy (passive case detection, with VRDL + FTA-ABS), considering the entrance and exit of inmates over a 40 year period. The model allows simulation of the implementation of a reverse algorithm for the current situation (rapid test + VDRL), different screening strategies (entry point, massive periodically; both with rapid test + VDRL) and treatment of detected cases. The parameters for the exploratory exercise were obtained from systematic searches of indexed and grey literature and field work (EQ-5D questionnaire application and key actors interviews). Probabilistic sensitivity analysis was conducted to account for uncertainty in relevant parameters.</jats:p> </jats:sec><jats:sec> <jats:title>Results</jats:title> <jats:p>The proposed framework allows the evaluation of different detection strategies. In this study, all the strategies were cost-effective in the baseline scenario when considering an ICER threshold of 1 Chilean GDP per capita (US$15,000). The strategies most likely to be cost-effective (over 80% probability) were: current situation with reverse algorithm, entry point screening and mass screening every two years; the latter was the most effective, achieving the lowest prevalence (0.7% and 1.7% over the period versus the 3% prevalence in the current situation).</jats:p> </jats:sec><jats:sec> <jats:title>Conclusions</jats:title> <jats:p>Mathematical modelling that considers the performance of different tests and detection strategies could be a useful tool for decision making. The exploratory results show the efficiency of adopting both the use of the rapid tests and performing active case detection to significantly reduce the burden of syphilis in Chilean prisons in the near future.</jats:p> </jats:sec>Scopus© Citations 5 1 - Some of the metrics are blocked by yourconsent settings
