    Psychological Profile of Trail Runners Associated With Running-Related Injuries: A Prospective Study

    <jats:sec> <jats:title>Background:</jats:title> <jats:p>Trail running has increased in popularity due to the benefits of physical activity in nature. However, trail running has an inherent risk of running-related injuries (RRI). It is known that athletes with certain psychological traits have a greater tendency to suffer injuries; however, this is unknown in trail runners. The main objective of this study was to identify trail runners’ psychological profiles and to compare the proportion of RRI across these profiles.</jats:p> </jats:sec> <jats:sec> <jats:title>Hypothesis:</jats:title> <jats:p>Trail runners with psychological profiles of high stress, precompetitive anxiety, mental fatigue, competitiveness, and poor sleep quality are at increased risk of RRI.</jats:p> </jats:sec> <jats:sec> <jats:title>Study Design:</jats:title> <jats:p>Prospective cohort study.</jats:p> </jats:sec> <jats:sec> <jats:title>Level of Evidence:</jats:title> <jats:p>Level 2.</jats:p> </jats:sec> <jats:sec> <jats:title>Methods:</jats:title> <jats:p>A Gaussian mixture model cluster analysis was performed on 202 trail runners (55.5% male; aged 38.7 [33.4-46.2] years) with psychological stress, cognitive and somatic anxiety, self-confidence, mental fatigue, sleep quality, and competitiveness measured 4 weeks before participating in a race. The proportion of RRI during the race was recorded and compared across clusters.</jats:p> </jats:sec> <jats:sec> <jats:title>Results:</jats:title> <jats:p> Overall RRI proportion during competition was 11.3% (n = 24). The most common RRI were muscle (41.7%) and tendon/bursa (16.7%) injuries, affecting primarily the knee (33.3%) and lower leg (20.8%). Five psychological profiles were identified. Cluster 1 (competitive runners with moderate psychological stress and mental fatigue, low sleep quality and anxiety, and high self-confidence) had a higher RRI proportion than Cluster 3 (similar traits but lower psychological stress, mental fatigue, and higher self-confidence; 21.2% vs 3.2%; <jats:italic toggle="yes">P</jats:italic>  = 0.02). </jats:p> </jats:sec> <jats:sec> <jats:title>Conclusion:</jats:title> <jats:p>Certain psychological profiles in trail runners were associated with higher RRI risk.</jats:p> </jats:sec> <jats:sec> <jats:title>Clinical Relevance:</jats:title> <jats:p>The medical team or trail running coaches should monitor runners with psychological profiles with higher psychological stress, mental fatigue, and cognitive anxiety, as well as lower self-confidence and sleep quality, to design strategies to reduce their risk of RRI.</jats:p> </jats:sec>

    Exploring contextual factors for management and prevention of running-related injuries: runners and experts’ perspectives

    Running is one of the most accessible and popular physical activities worldwide; however, injuries are the main barrier to sustaining running practice. While quantitative studies have explored prevalence and risk factors, a critical gap exists in understanding subjective experiences, perceptions and contextual influences on injury management and prevention. This qualitative study aimed to explore the perspectives of runners and experts regarding injury perception, management and prevention, as well as the contextual influence of these processes. Using a secondary data analysis approach, this study drew from qualitative semistructured interviews with Chilean runners (n=15) and running experts (n=6). Thematic analysis, guided by an interpretivist approach, uncovered intrinsic factors (identity, motivation, stress and self-learning) and extrinsic factors (environment, information sources, marketing, peer advice, professional guidance, racing, stereotype and clothing) that shaped runners’ behaviours. Less experienced runners associated injury risk with asphalt surfaces, faced challenges in discerning online information reliability and found motivation in peer advice. Experts emphasised the multifactorial nature of running-related injuries, including previous injuries and training-related factors. Both groups acknowledged a global tendency among runners to resist rest when discomfort arose. This research contributed to a nuanced understanding of injury perception, management and prevention, bridging scientific knowledge with individual experiences. Clinicians may use this information to enhance the therapy alliance and set realistic expectations about the runner’s rehabilitation process.

    How fractal complexity distorts distance and elevation gain in trail and mountain running: The case for course measurement standardization

    This study investigates how fractal complexity affects Trail and mountain running (TMR) race course measurements at varying GPS resolutions and emphasizes the need for standardized course measurement protocols. GPX files from 34 UTMB World Series race courses, including final events in Chamonix, were analyzed. Horizontal distance, elevation gain, km-effort, and fractal complexity were computed at varying GPS spatial resolutions (0.2–100 m). Elevation data were refined using a 20-cm resolution Digital Elevation Model (DEM) to ensure consistency across the dataset. The courses were systematically resampled and compared to assess the effects of spatial resolution on race measurements and classifications. The findings reveal that a decrease in GPS spatial resolution significantly reduces measured distances and elevation gains. Discrepancies in kilometer-effort reached up to 14% (mean = 7.0%, SD = 3.8%), horizontal distance up to 6.3% (mean = 2.9%, SD = 1.5%), and elevation gain up to 32% (mean = 14.0%, SD = 9.5%). Adopting a 1-m resolution, chosen for its practical balance between capturing terrain complexity at a human scale and computational efficiency, would enhance the reliability of distance, elevation gain, and km-effort calculations, ensuring fairer race classifications and improved comparability across events.

    Implementation of physiotherapy telerehabilitation before and post Covid-19 outbreak: A comparative narrative between South American countries and Australia

    <jats:p>The continuous development in telecommunication tech­nologies has created opportunities for health professionals to optimise healthcare delivery by adopting digital tools into rehabilitation programs (i.e., telerehabilitation). These tech­nological advances, along with the demographic and social characteristics of each country, have made the implementa­tion of telerehabilitation a disparate process across regions. We have gathered the experience of four countries (Australia, Chile, Brazil, and Colombia) in two different regions (Ocea­nia and South America) to recompile the history pre- and post-Covid-19 outbreak until January of 2021, the barriers to, and facilitators of telerehabilitation, and outline the future challenges for these countries.</jats:p>

    Frequency of Injury and Illness in the Final 4 Weeks before a Trail Running Competition

    <jats:p>We aimed to (i) determine self-reported injury and illness frequency in trail runners 4 weeks preceding competition; (ii) compare athletes with and without injury/illness by sex, age, body mass index (BMI) and competition distance; (iii) describe mechanism of injury, anatomical region (injury)/organ system (illness) involved, consequences of injury on preparation and self-perception of injury severity; (iv) compare anatomical region (injury) and organ system (illness) by sex. A total of 654 trail runners (age 36.2, IQR 30.6–43.0; 36.9% females) participated in this retrospective cross-sectional study by completing a self-reported questionnaire. Injury and illness frequency rates were 31.3% (n = 205, CI:27.7–35.0%) and 22.3% (n = 146, CI:19.1–25.7%), respectively. No significant difference was found between injured vs. non-injured or ill vs. non-ill study participants by sex, age, BMI and competition distance. Regarding injuries, gradual onset (41.6%) and knee (33.2%) were the most indicated mechanism and anatomical region of injury. At least 85.4% of trail runners changed their training following injury and 79% indicated that their injury would affect their competition performance. Regarding illness, the respiratory tract was the most frequent organ system involved (82.9%). Male and female participants reported similar proportions of anatomical regions (injury) and organ systems (illness) affected. These results could help to generate education strategies and appropriate medical support before and during these competitions.</jats:p>

    Aim: To analyze the control of training load for an acclimatization strategy employed by two amateur athletes (female and male) prior to a successful ascent to 5460 meters. Methodology: Baseline heart rate variability (HRV) measurements were taken for 195 consecutive days. Four blood samples were taken the week before the main event (Pre), pre-ascent (Pre A), post-ascent (Post A), and one-week post-ascent (Post C). Results: Daily morning HRV recordings were useful for monitoring the state of sympathetic-parasympathetic balance in mountaineers prior to tackling training sessions. Both subjects had increased leukocytes and neutrophil levels, exhibiting variations over 60% between pre-A and post-B conditions. Erythrocytes, hematocrit, and haemoglobin decreased after acute exposure to 5460 m, coinciding with changes in platelet levels between both subjects (-3% and -51%). Conclusions: Daily morning and 5-min HRV recordings are a useful way for monitoring the state of sympathetic-parasympathetic balance. Significant changes were observed in some white blood cells.</jats:p> <jats:p xml:lang="es">Objetivo: Analizar el control de la carga de entrenamiento en la estrategia de aclimatación empleada por dos atletas amateur (mujer y hombre) antes de una ascensión exitosa a 5460 metros. Metodología: Durante 195 días consecutivos se realizaron mediciones basales de variabilidad de la frecuencia cardiaca (VFC). Se obtuvieron cuatro muestras de sangre durante la semana previa al evento principal (Pre), inmediatamente antes de la ascensión (Pre A), después (Post A) y una semana de recuperación (Post C). Resultados: Los registros diarios de VFC matutinos parecen ser una forma útil para monitorizar el estado de equilibrio simpático-parasimpático en montañistas antes de abordar las sesiones de entrenamiento. Se observó que ambos sujetos aumentaron sus niveles de leucocitos y neutrófilos exhibiendo variaciones sobre 60% entre las condiciones pre A y post B. Eritrocitos, hematocrito y hemoglobina presentaron tras la exposición aguda a 5460 m disminuciones, lo que coincide con las diferencias porcentuales en plaquetas entre ambos sujetos (-3% y -51%). Conclusiones: Los registros diarios de VFC matutinos y de 5 minutos de duración parecen ser una forma útil para monitorizar el estado de equilibrio simpático-parasimpático. Se observaron cambios significativos en algunas células de la serie blanca.

    The Biomechanics Research and Innovation Challenge: Development, Implementation, Uptake, and Reflections on the Inaugural Program

    <jats:p>Biomechanics as a discipline is ideally placed to increase awareness and participation of girls and women in science, technology, engineering, and mathematics. A nationwide Biomechanics and Research Innovation Challenge (BRInC) centered on mentoring and role modeling was developed to engage high school girls (mentees) and early-mid-career women (mentors) in the field of biomechanics through the completion of a 100-day research and/or innovation project. This manuscript describes the development, implementation, and uptake of the inaugural BRInC program and synthesizes the research and innovation projects undertaken, providing a framework for adoption of this program within the global biomechanics community. Eighty-seven high school girls in years 9 and 10 (age range: 14–16 y) were mentored in teams (n = 17) by women in biomechanics (n = 24). Using a design thinking approach, teams generated solutions to biomechanics-based problem(s)/research question(s). Eight key reflections on program strengths, as well as areas for improvement and planned changes for future iterations of the BRInC program, are outlined. These key reflections highlight the innovation, impact, and scalability of the program; the importance of a program framework and effective communication tools; and implementation of strategies to sustain the program as well as the importance of diversity and building a sense of community.</jats:p>
