    Resilience of mobility network to dynamic population response across COVID-19 interventions: Evidences from Chile

    The COVID-19 pandemic highlighted the importance of non-traditional data sources, such as mobile phone data, to inform effective public health interventions and monitor adherence to such measures. Previous studies showed how socioeconomic characteristics shaped population response during restrictions and how repeated interventions eroded adherence over time. Less is known about how different population strata changed their response to repeated interventions and how this impacted the resulting mobility network. We study population response during the first and second infection waves of the COVID-19 pandemic in Chile and Spain. Via spatial lag and regression models, we investigate the adherence to mobility interventions at the municipality level in Chile, highlighting the significant role of wealth, labor structure, COVID-19 incidence, and network metrics characterizing business-as-usual municipality connectivity in shaping mobility changes during the two waves. We assess network structural similarities in the two periods by defining mobility hotspots and traveling probabilities in the two countries. As a proof of concept, we simulate and compare outcomes of an epidemic diffusion occurring in the two waves. While differences exist between factors associated with mobility reduction across waves in Chile, underscoring the dynamic nature of population response, our analysis reveals the resilience of the mobility network across the two waves. We test the robustness of our findings recovering similar results for Spain. Finally, epidemic modeling suggests that historical mobility data from past waves can be leveraged to inform future disease spatial invasion models in repeated interventions. This study highlights the value of historical mobile phone data for building pandemic preparedness and lessens the need for real-time data streams for risk assessment and outbreak response. Our work provides valuable insights into the complex interplay of factors driving mobility across repeated interventions, aiding in developing targeted mitigation strategies.

    Los inicios de la enseñanza de pediatría en Chile y la fundación de hospitales de niños

    <jats:p>El objetivo del presente manuscrito es revisar antecedentes históricos previos a la creación de la Sociedad Chilena de Pediatría (SOCHIPE) en el año 1922, con el propósito de reflexionar sobre su impacto en la profesionalización de la pediatría nacional, especialmente en la salud y sobrevida de los niños. Se revisaron documentos de la Junta de Beneficencia, libros y estudios especializados de la época. Hasta principios del siglo XX, el país ostentaba una de las tasas de mortalidad infantil más elevadas del mundo, y contaba con precarias instalaciones para enfrentarlas. En aquellos años, el único establecimiento cerrado para la atención de infantes era la Casa de Huérfanos, que estaba al cuidado de religiosas con apoyo de médicos; es ahí donde se desarrollaron los primeros cursos teórico-prácticos de la especialidad. Presionados por la alta mortalidad infantil y las mortíferas epidemias, la Junta de Beneficencia vio la urgencia de mejorar las condiciones higiénicas y crear instalaciones para el cuidado de los niños. El manuscrito aborda el surgimiento de espacios encargados del resguardo de la salud de los menores de cinco años, desde la creación de la cátedra de pediatría y del primer hospital de niños, el Hospital Roberto del Río, lo que marcó el inicio de las políticas de salud pública infantiles.</jats:p>

    The Work, Economic, and Remittance Stress and Distress of the COVID-19 Pandemic Containment Policies: The Case of Venezuelan Migrants in Argentina and Chile

    <jats:p>According to the social stress process model, global crises are macro-level stressors that generate physiological stress and psychological distress. However, existing research has not identified immigrants’ COVID-19 containment policy stressors or examined the social stress of sending remittances amid crises. Drawing on in-depth longitudinal interviews with 46 Venezuelan immigrants—half before and half during the pandemic—in Chile and Argentina, we identified the COVID-19 containment policies’ stressors. We focused on Venezuelan immigrants because they constitute one of the largest internationally displaced populations, with most migrating within South America. We found that the governmental COVID-19 containment measures in both countries generated four stressors: employment loss, income loss, devaluation of employment status, and inability to send needed remittances. Moreover, sending remittances helped some migrants cope with concerns about loved ones in Venezuela. However, sending remittances became a social stressor when immigrants struggled to simultaneously sustain their livelihoods and send financial support to relatives experiencing hardships in Venezuela. For some immigrants, these adversities generated other stressors (e.g., housing instability) and symptoms of anxiety and depression. Broadly, for immigrants, the stressors of global crises transcend international borders and generate high stress, which strains their psychological well-being.</jats:p>

    A multispecies outbreak of carbapenem-resistant bacteria harboring the blaKPC gene in a non-classical transposon element

    <jats:title>Abstract</jats:title><jats:sec> <jats:title>Background</jats:title> <jats:p><jats:italic>Klebsiella pneumoniae</jats:italic> is the most frequent KPC-producing bacteria. The <jats:italic>bla</jats:italic><jats:sub>KPC</jats:sub> gene is frequently embedded in Tn4401 transposon, and less frequently in non-Tn4401 elements (NTE<jats:sub>KPC</jats:sub>) variants I-III. The first case of KPC in the UC-CHRISTUS Clinical Hospital was detected in <jats:italic>Pseudomonas aeruginosa</jats:italic>. Soon after this event, KPC was detected in 2 additional <jats:italic>Pseudomonas aeruginosa</jats:italic>, 3 <jats:italic>Escherichia coli</jats:italic>, 3 <jats:italic>Enterobacter cloacae</jats:italic>, 3 <jats:italic>Klebsiella pneumoniae,</jats:italic> and 1 <jats:italic>Citrobacter freundii</jats:italic>, isolated from 6 different patients. We aimed to elucidate the possible mechanisms of genetic transfer and dissemination of the <jats:italic>bla</jats:italic><jats:sub>KPC</jats:sub> gene among isolates of this multispecies outbreak. A molecular epidemiology analysis of the above mentioned clinical isolates (<jats:italic>n</jats:italic> = 13) through Multi-Locus Sequence Typing, plasmid analysis, Pulsed-Field Gel-Electrophoresis, and Whole-genome sequencing (WGS) was performed.</jats:p> </jats:sec><jats:sec> <jats:title>Results</jats:title> <jats:p>High-risk sequence types were found: <jats:italic>K. pneumoniae</jats:italic> ST11, <jats:italic>P. aeruginosa</jats:italic> ST654, and <jats:italic>E. cloacae</jats:italic> ST114. All enterobacterial isolates were not clonal except for 3 <jats:italic>E. coli</jats:italic> isolated from the same patient. WGS analysis in 6 enterobacterial isolates showed that 4 of them had <jats:italic>bla</jats:italic><jats:sub>KPC</jats:sub> embedded in a novel variant of NTE<jats:sub>KPC</jats:sub> designated NTE<jats:sub>KPC</jats:sub>-IIe. Upstream of <jats:italic>bla</jats:italic><jats:sub>KPC</jats:sub> gene there was a 570 pb truncated <jats:italic>bla</jats:italic><jats:sub>TEM-1</jats:sub> gene followed by an insertion sequence that was 84% similar to ISEc63, a 4473 bp element of the Tn3 family. Downstream the <jats:italic>bla</jats:italic><jats:sub>KPC</jats:sub> gene there was a truncated ISKpn6 gene, and the inverted repeat right sequence of Tn4401. The ISec63-like element together with the <jats:italic>bla</jats:italic><jats:sub>KPC</jats:sub> gene plus Tn4401 remnants were inserted in the Tra operon involved in conjugative transfer of the plasmid. This NTE was carried in a broad host-range IncN plasmid. <jats:italic>P. aeruginosa</jats:italic> isolates carried <jats:italic>bla</jats:italic><jats:sub>KPC</jats:sub> gene embedded in a typical Tn4401b transposon in a different plasmid, suggesting that there was no plasmid transfer between <jats:italic>Enterobacteriaceae</jats:italic> and <jats:italic>P. aeruginosa</jats:italic> as initially hypothesized.</jats:p> </jats:sec><jats:sec> <jats:title>Conclusions</jats:title> <jats:p>Most enterobacterial isolates had <jats:italic>bla</jats:italic><jats:sub>KPC</jats:sub> embedded in the same NTE<jats:sub>KPC</jats:sub>-IIe element, suggesting that this multispecies KPC outbreak was due to horizontal gene transfer rather than clonal spread. This poses a greater challenge to infection control measures often directed against containment of clonal spread.</jats:p> </jats:sec>

    Rickettsial infections: A blind spot in our view of neglected tropical diseases

    <jats:p>Rickettsial diseases are a group of vector-borne bacterial infections that cause acute febrile illness with potentially severe or fatal complications. These vector-borne diseases are prevalent in tropical and subtropical regions worldwide and disproportionately affect poorer communities but are scientifically underrecognized. Despite this, they are not included in the World Health Organization’s list of neglected tropical diseases nor were they mentioned in Peter Hotez’s recent reflections on “What constitutes a neglected tropical disease?” in <jats:italic>PLOS Neglected Tropical Diseases</jats:italic> [1]. Here we present the case that rickettsial infections, as an overlooked cause of morbidity, mortality, and economic losses in marginalized populations, should be recognized as neglected tropical diseases. We describe how this oversight is the result of a number of factors and how it negatively impacts patient outcomes. We then propose measures to address the neglect of rickettsial infections in both scientific research and public health interventions.</jats:p>
