    Theta and alpha oscillations may underlie improved attention and working memory in musically trained children

    <jats:title>Abstract</jats:title><jats:p><jats:bold>Introduction</jats:bold>: Attention and working memory are key cognitive functions that allow us to select and maintain information in our mind for a short time, being essential for our daily life and, in particular, for learning and academic performance. It has been shown that musical training can improve working memory performance, but it is still unclear if and how the neural mechanisms of working memory and particularly attention are implicated in this process. In this work, we aimed to identify the oscillatory signature of bimodal attention and working memory that contributes to improved working memory in musically trained children.</jats:p><jats:p><jats:bold>Materials and methods</jats:bold>: We recruited children with and without musical training and asked them to complete a bimodal (auditory/visual) attention and working memory task, whereas their brain activity was measured using electroencephalography. Behavioral, time–frequency, and source reconstruction analyses were made.</jats:p><jats:p><jats:bold>Results</jats:bold>: Results showed that, overall, musically trained children performed better on the task than children without musical training. When comparing musically trained children with children without musical training, we found modulations in the alpha band pre‐stimuli onset and the beginning of stimuli onset in the frontal and parietal regions. These correlated with correct responses to the attended modality. Moreover, during the end phase of stimuli presentation, we found modulations correlating with correct responses independent of attention condition in the theta and alpha bands, in the left frontal and right parietal regions.</jats:p><jats:p><jats:bold>Conclusions</jats:bold>: These results suggest that musically trained children have improved neuronal mechanisms for both attention allocation and memory encoding. Our results can be important for developing interventions for people with attention and working memory difficulties.</jats:p>

    Componentes ético-clínicos presentes en la indicación de tratamiento en mujeres con cáncer de mama según edad de diagnóstico

    <jats:p>Objetivo: Evaluar si existen diferencias basadas en la edad en la aproximación diagnóstica y de tratamiento del cáncer de mama. Se analizan aspectos éticos, como justicia, beneficencia y autonomía, en relación a la toma de decisiones.Material y método: Estudio descriptivo cuantitativo y corresponde a un análisis retrospectivo de la base de datos del Centro de la Mama de Clínica Alemana de Santiago, en la que se analiza información de las mujeres de 70 años o más en relación a motivo consulta, estadio de la enfermedad al momento del diagnóstico y tratamientos recibidos y se la compara con la obtenida del grupo de mujeres menores de 70 años.Resultados: No hay diferencia en la oportunidad de la consulta, en el acceso al diagnóstico ni en el tipo de cirugía entre los dos grupos. Existen diferencias significativas en relación a la indicación de tratamientos adyuvantes como quimioterapia, radioterapia y hormonoterapia. El grupo de mujeres mayores de 70 años reciben menos terapias que las mujeres de menor edad. No existe información respecto a los motivos que expliquen esta diferencia.Discusión: Actualmente mujeres mayores se encuentran en buenas condiciones generales, con una expectativa de vida mayor a 75 años. Es importante tratar sus enfermedades sin limitar a priori el acceso a protocolos de tratamiento, evitando la discriminación por edad.Conclusión: Se propone incorporar una evaluación geriátrica protocolizada con el  objetivo de mejorar la  atención de este grupo etario, preservando de esta manera los principios de justicia, beneficencia y autonomía en pacientes mayores.</jats:p>

    Critical illness myopathy and trajectory of recovery in acute kidney injury requiring continuous renal replacement therapy: a prospective observational trial protocol

    <jats:sec><jats:title>Introduction</jats:title><jats:p>Acute kidney injury requiring renal replacement therapy (AKI-RRT) is common in the intensive care unit (ICU) and is associated with significant morbidity and mortality. Continuous RRT (CRRT) non-selectively removes large amounts of amino acids from plasma, lowering serum amino acid concentrations and potentially depleting total-body amino acid stores. Therefore, the morbidity and mortality associated with AKI-RRT may be partly mediated through accelerated skeletal muscle atrophy and resulting muscle weakness. However, the impact of AKI-RRT on skeletal muscle mass and function during and following critical illness remains unknown. We hypothesise that patients with AKI-RRT have higher degrees of acute muscle loss than patients without AKI-RRT and that AKI-RRT survivors are less likely to recover muscle mass and function when compared with other ICU survivors.</jats:p></jats:sec><jats:sec><jats:title>Methods and analysis</jats:title><jats:p>This protocol describes a prospective, multicentre, observational trial assessing skeletal muscle size, quality and function in ICU patients with AKI-RRT. We will perform musculoskeletal ultrasound to longitudinally evaluate rectus femoris size and quality at baseline (within 48 hours of CRRT initiation), day 3, day 7 or at ICU discharge, at hospital discharge, and 1–3 months postdischarge. Additional skeletal muscle and physical function tests will be performed at hospital discharge and postdischarge follow-up. We will analyse the effect of AKI-RRT by comparing the findings in enrolled subjects to historical controls of critically ill patients without AKI-RRT using multivariable modelling.</jats:p></jats:sec><jats:sec><jats:title>Ethics and dissemination</jats:title><jats:p>We anticipate our study will reveal that AKI-RRT is associated with greater degrees of muscle loss and dysfunction along with impaired postdischarge recovery of physical function. These findings could impact the in-hospital and postdischarge treatment plan for these patients to include focused attention on muscle strength and function. We intend to disseminate findings to participants, healthcare professionals, the public and other relevant groups via conference presentation and publication without any publication restrictions.</jats:p></jats:sec><jats:sec><jats:title>Trial registration number</jats:title><jats:p><jats:ext-link xmlns:xlink="http://www.w3.org/1999/xlink" ext-link-type="clintrialgov" xlink:href="NCT05287204">NCT05287204</jats:ext-link>.</jats:p></jats:sec>
