    Case Report: Acute Onset Fear of Falling and Treatment With “Cognitive Physical Therapy”

    <jats:p>Fear of falling (FoF) is prevalent in older adults, especially those with previous falls, and typically starts insidiously. We present a 78-year-old woman with an abrupt onset FoF and no history of falls, balance problems, vertigo, oscillopsia, psychiatric or psychological issues to account for this. These cognitive changes led to a behavioural alteration of her gait that became slow and wide-based, with her gaze fixed on the floor. She began a tailored program of “Cognitive Physical Therapy (CPT)” combining cognitive behavioural therapy (CBT) and physical rehabilitation. 1 month later her 6 m walk time and steps were reduced by a 25 and 35%, respectively, and the stride length increased by 34%, with further improvement 2 months later. We postulate that the abrupt onset of symptoms triggered a central shift toward postural hypervigilance and anxiety, suppression of anticipatory (feed forward) postural adjustments (APA) leading to FoF. CPT improved objective gait parameters related to FoF and reduced postural anxiety suggesting that early diagnosis and prompt treatment may avoid chronic symptoms and social isolation.</jats:p>

    Bone mineral density in young Chilean patients with type 1 diabetes mellitus

    In this study, our aim was to analyze bone mineral density (BMD) in patients with type 1 diabetes mellitus (T1DM) and compare them with a healthy reference population; in addition, we aimed to observe the association between BMD and the following variables: age at onset, disease duration, metabolic control, pubertal stage, level of physical activity, clinical parameters and nutrient intake.</jats:p><jats:p>A total of 30 patients with T1DM were included in the study. BMD was determined using dual-energy X-ray densitometry (DXA). Participants with a z-score of values ≥–1 were accepted as normal; BMDs between –2 and –1 were defined as being in the low range of normality; ≤–2 were defined as having low BMD. The 25-hydroxy vitamin D level was classified as sufficient (30–100 ng/mL), insufficient (20–30 ng/mL), and deficient (&lt;20 ng/mL).</jats:p><jats:p>The percentages of patients with deficient and insufficient 25(OH) vitamin D levels were 50% and 45.8%, respectively. Lumbar spine (LS2–LS4) BMD, total body (TB) BMD and femoral neck (FN) BMD were found in the normal range for more than 80% of the subjects, with no significant differences due to gender. No strong correlations between clinical variables, biochemical parameters and nutrient intake were observed; however, a moderate positive correlation was found between serum calcium and LS2–LS4 BMD (p&lt;0.05). Regression analysis showed that serum calcium, duration of diabetes and intake of sodium and protein are significant factors in determining LS2–LS4 BMD and TB BMD.</jats:p><jats:p>Patients with T1DM had a normal mean BMD at all sites evaluated, except for two patients who had low BMD at the lumbar spine. More than 95% of patients had insufficient or deficient vitamin D levels. With respect to all the variables studied, serum calcium presented the highest significant correlation with LS2–LS4 BMD.

    Deficiencia e insuficiencia de vitamina D en lactantes sanos recibiendo suplementación estándar

    <jats:p>La vitamina D (VD) es indispensable para el metabolismo del calcio y fósforo, su deficiencia puede causar raquitismo. En Chile se suplementa con 400 UI /diarias desde el primer mes hasta el año de vida. Objetivo: Describir las concentraciones plasmáticas de VD en lactantes sanos suplementados y secundariamente, evaluar la asociación del estatus de VD con estacionalidad y estado nutricional. Sujetos y Método: Estudio de corte transversal. Se evaluó a lactantes con lactancia materna exclusiva o mixta que acudían a controles pediátricos mensuales y recibían suplementación de VD a dosis de 400 UI diarias. A los 6 meses de edad se midió la concentración plasmática de VD, el peso y la talla. Posteriormente se clasificó el estado nutricional (peso/edad y peso/talla) según los referentes de la OMS. De acuerdo a la concentración plasmática de VD, se definió como normal ≥ 30 ng/ml, insuficiencia si esta fue entre 20 y 29 ng/ml y deficiencia &lt; 20 ng/ml. Resultados: Se estudiaron 40 lactantes, 40% tuvieron concentraciones de VD insuficientes y 40% deficientes. En el modelo de regresión lineal múltiple, las variables que se asociaron, de modo significativo, con una menor concentración de VD fueron: el periodo del año invierno-primavera (p = 0,007) y el riesgo de desnutrición (p = 0,038). Conclusiones: La frecuencia de deficiencia e insuficiencia de VD es elevada en nuestra población suplementada, dicho déficit es mayor durante el invierno y la primavera y en sujetos con riesgo de desnutrición.</jats:p>
