    Head-to-head comparison of CAMPYAIR aerobic culture medium versus standard microaerophilic culture for Campylobacter isolation from clinical samples

    <jats:p><jats:italic>Campylobacter</jats:italic> spp. are considered the most frequent cause of acute gastroenteritis worldwide. However, outside high-income countries, its burden is poorly understood. Limited published data suggest that <jats:italic>Campylobacter</jats:italic> prevalence in low- and middle-income countries is high, but their reservoirs and age distribution are different. Culturing <jats:italic>Campylobacter</jats:italic> is expensive due to laboratory equipment and supplies needed to grow the bacterium (e.g., selective culture media, microaerophilic atmosphere, and a 42°C incubator). These requirements limit the diagnostic capacity of clinical laboratories in many resource-poor regions, leading to significant underdiagnosis and underreporting of isolation of the pathogen. CAMPYAIR, a newly developed selective differential medium, permits <jats:italic>Campylobacter</jats:italic> isolation without the need for microaerophilic incubation. The medium is supplemented with antibiotics to allow <jats:italic>Campylobacter</jats:italic> isolation in complex matrices such as human feces. The present study aims to evaluate the ability of the medium to recover <jats:italic>Campylobacter</jats:italic> from routine clinical samples. A total of 191 human stool samples were used to compare the ability of CAMPYAIR (aerobic incubation) and a commercial <jats:italic>Campylobacter</jats:italic> medium (CASA, microaerophilic incubation) to recover <jats:italic>Campylobacter</jats:italic>. All <jats:italic>Campylobacter</jats:italic> isolates were then identified by MALDI-TOF MS. CAMPYAIR showed sensitivity and specificity values of 87.5% (95% CI 47.4%–99.7%) and 100% (95% CI 98%–100%), respectively. The positive predictive value of CAMPYAIR was 100% and its negative predictive value was 99.5% (95% CI 96.7%–99.9%); Kappa Cohen coefficient was 0.93 (95% CI 0.79–1.0). The high diagnostic performance and low technical requirements of the CAMPYAIR medium could permit <jats:italic>Campylobacter</jats:italic> culture in countries with limited resources.</jats:p>

    Enjuagues bucales con carbohidratos y su uso en deportistas. Resumen de revisiones sistemáticas (Carbohydrate mouthwashes and their use in athletes. Overview of systematic reviews)

    <jats:p>Objetivo; Analizar los efectos del enjuague bucal con carbohidratos en deportistas. Material y Métodos; Se realizó una búsqueda en la revista Retos, además de Medline / PubMed, Web of Science, Springer, Scopus, Europe PMC, SPOLIT, SPORTDiscus, SciELO, Epistemonikos y Trip database. Las búsquedas se realizaron entre los años 2018 y 2022 utilizando la estrategia “carbohydrate AND mouth rinse AND sports”, seleccionándose solo revisiones sistemáticas con metaanálisis. Resultados; Se identificaron 318 registros de los cuales luego de aplicar los criterios de elegibilidad se analizaron tres, cuyos resultados mostraron posibles mejoras sobre la potencia media de salida (Diferencia media estandarizada (DME) = 0,25, IC 95%; 0,04 a 0,46), rendimiento deportivo (DME = 0,15, IC 95%; 0,04 a 0,27) y la resistencia muscular hasta el fallo (Diferencia media (DM) = 1,24, IC 95%; 0,70 a 1,77), mientras que el tiempo contrarreloj (DME = -0,13, IC 95; -0,36 a  0,10) y la fuerza máxima muscular  (DM = 0,25, IC 95%; -1,81 a 2,32) no muestran mejoras significativas. Conclusión; La suministración de un protocolo de enjuagues bucales con carbohidratos en forma constante puede constituir una ayuda ergogénica. Sin embargo, estos efectos aún son controvertidos. Objective; To analyze the effects of mouthwash with carbohydrates in athletes. Material and methods; A search was carried out in the journal Retos, in addition to Medline / PubMed, Web of Science, Springer, Scopus, Europe PMC, SPOLIT, SPORTDiscus, SciELO, Epistemonikos and Trip database. The searches were carried out between the years 2018 and 2022 using the strategy "carbohydrates AND mouthwash AND sports", selecting only systematic reviews with meta-analysis. Results; 318 records were identified, of which after applying the eligibility criteria, three were analyzed, which results showed possible improvements on the mean power output (Standardized Mean Difference (SMD) = 0.25, 95% CI; 0.04 to 0 .46), athletic performance (SMD = 0.15, 95% CI, 0.04 to 0.27), and muscular endurance to failure (Mean Difference (MD) = 1.24, 95% CI, 0.70 to 1.77), while time trial (SMD = -0.13, 95% CI; -0.36 to 0.10) and maximum muscle strength (MD = 0.25, 95% CI; -1, 81 to 2.32) does not show significant improvements. Conclution; The administration of a protocol of mouth rinses with carbohydrates in a constant way could constitute an ergogenic aid. However, these effects are still controversial.     Deficiencia e insuficiencia de vitamina D en lactantes sanos recibiendo suplementación estándar

    <jats:p>La vitamina D (VD) es indispensable para el metabolismo del calcio y fósforo, su deficiencia puede causar raquitismo. En Chile se suplementa con 400 UI /diarias desde el primer mes hasta el año de vida. Objetivo: Describir las concentraciones plasmáticas de VD en lactantes sanos suplementados y secundariamente, evaluar la asociación del estatus de VD con estacionalidad y estado nutricional. Sujetos y Método: Estudio de corte transversal. Se evaluó a lactantes con lactancia materna exclusiva o mixta que acudían a controles pediátricos mensuales y recibían suplementación de VD a dosis de 400 UI diarias. A los 6 meses de edad se midió la concentración plasmática de VD, el peso y la talla. Posteriormente se clasificó el estado nutricional (peso/edad y peso/talla) según los referentes de la OMS. De acuerdo a la concentración plasmática de VD, se definió como normal ≥ 30 ng/ml, insuficiencia si esta fue entre 20 y 29 ng/ml y deficiencia &lt; 20 ng/ml. Resultados: Se estudiaron 40 lactantes, 40% tuvieron concentraciones de VD insuficientes y 40% deficientes. En el modelo de regresión lineal múltiple, las variables que se asociaron, de modo significativo, con una menor concentración de VD fueron: el periodo del año invierno-primavera (p = 0,007) y el riesgo de desnutrición (p = 0,038). Conclusiones: La frecuencia de deficiencia e insuficiencia de VD es elevada en nuestra población suplementada, dicho déficit es mayor durante el invierno y la primavera y en sujetos con riesgo de desnutrición.</jats:p>
