    “Hasta que me embaracé no conocí matrona”: salud sexual y reproductiva de jóvenes mujeres mapuche, Chile

    <jats:p>Resumen Este estudio pretende explorar la prevención y promoción de salud sexual y reproductiva en jóvenes mapuche de entre 18 y 24 años, indagando en las relaciones que establecen con el sistema de salud biomédico y en los desafíos para la inclusión de la interculturalidad en las prestaciones dirigidas a esta población. Se trata de una investigación cualitativa de estudio de caso. Se aplicaron 32 entrevistas en profundidad a jóvenes mapuche de sectores rurales y urbanos de la región de la Araucanía, Chile. Se identifica la persistencia un abordaje sanitario que carece de herramientas interculturales y de enfoque de derechos sexuales y reproductivos. Las jóvenes reportan falta de acceso a educación sexual tanto por la distancia que establecen con los servicios biomédicos de atención primaria como por las dificultades de hablar de sexualidad con los adultos de sus comunidades. Se concluye que persisten inequidades en el derecho a la salud sexual y reproductiva de jóvenes indígenas, especialmente en mujeres. Es necesario incorporar el enfoque intercultural y de derechos en la formulación de políticas públicas para esta población. Tales intervenciones requieren ser diseñadas e implementadas involucrando tanto a la población joven como a los agentes de salud de sus comunidades de origen.</jats:p>

    Global research priorities for COVID-19 in maternal, reproductive and child health: Results of an international survey

    <jats:sec id="sec001"> <jats:title>Background</jats:title> <jats:p>The World Health Organization’s “<jats:italic>Coordinated Global Research Roadmap</jats:italic>: <jats:italic>2019 Novel Coronavirus</jats:italic>” outlined the need for research that focuses on the impact of COVID-19 on pregnant women and children. More than one year after the first reported case significant knowledge gaps remain, highlighting the need for a coordinated approach. To address this need, the Maternal, Newborn and Child Health Working Group (MNCH WG) of the COVID-19 Clinical Research Coalition conducted an international survey to identify global research priorities for COVID-19 in maternal, reproductive and child health.</jats:p> </jats:sec> <jats:sec id="sec002"> <jats:title>Method</jats:title> <jats:p>This project was undertaken using a modified Delphi method. An electronic questionnaire was disseminated to clinicians and researchers in three different languages (English, French and Spanish) via MNCH WG affiliated networks. Respondents were asked to select the five most urgent research priorities among a list of 17 identified by the MNCH WG. Analysis of questionnaire data was undertaken to identify key similarities and differences among respondents according to questionnaire language, location and specialty. Following elimination of the seven lowest ranking priorities, the questionnaire was recirculated to the original pool of respondents. Thematic analysis of final questionnaire data was undertaken by the MNCH WG from which four priority research themes emerged.</jats:p> </jats:sec> <jats:sec id="sec003"> <jats:title>Results</jats:title> <jats:p>Questionnaire 1 was completed by 225 respondents from 29 countries. Questionnaire 2 was returned by 49 respondents. The four priority research themes which emerged from the analysis were 1) access to healthcare during the COVID-19 pandemic, 2) the direct and 3) indirect effects of COVID-19 on pregnant and breastfeeding women and children and 4) the transmission of COVID-19 and protection from infection.</jats:p> </jats:sec> <jats:sec id="sec004"> <jats:title>Conclusion</jats:title> <jats:p>The results of these questionnaires indicated a high level of concordance among continents and specialties regarding priority research themes. This prioritized list of research uncertainties, developed to specifically highlight the most urgent clinical needs as perceived by healthcare professionals and researchers, could help funding organizations and researchers to answer the most pressing questions for clinicians and public health professionals during the pandemic. It is hoped that these identified priority research themes can help focus the discussion regarding the allocation of limited resources to enhance COVID-19 research in MNCH globally.</jats:p> </jats:sec>

    Latin American Youths’ Migration Journeys and Settlement in the Tarapacá Region in Chile: Implications for Sexual and Reproductive Health
    <jats:p>The sexual and reproductive health of young migrants has not been sufficiently addressed in mobility studies. In this article, we dwell on some aspects of this issue in the migration process of Latin American youth. We conducted a qualitative study in the region of Tarapacá, Chile, carrying out in-depth interviews with key informants, health staff and young migrants between 18 and 25 years old. The results show some motivations to migrate related to sexual and reproductive health: young pregnant women, LGBTQI+ and HIV-positive people seeking access to health care and social contexts of reduced gender discrimination. During the migration process, young people are exposed to various kinds of sexual violence, and in their settlement in Chile, to situations of racism, stigma and discrimination in society as a whole and in access to and during sexual and reproductive health care. Health care for young migrants is mainly focused on maternal care and reproductive issues, while sexual health as a whole is disregarded. We argue that sexual health must be addressed as a central dimension of the lived experiences of young migrants, and that the social, cultural and structural factors that undermine their sexual and reproductive health must be addressed in order to provide culturally competent health services.</jats:p>

    Strengths and challenges of a school-based sexual and reproductive health program for adolescents in Chile

    <jats:sec id="sec001"> <jats:title>Background</jats:title> <jats:p>Adolescent sexual and reproductive health services in Chile have been primarily provided through health centers. Although some school-based initiatives have been implemented, to date, these have not been assessed. This study aims to identify strengths and challenges of the affectivity and sexuality component of the school-based 3A Program, a health program which seeks to prevent risk behaviors and promote healthy lifestyle habits within public schools (addressing health topics which in Spanish begin with the letter ‘A’, hence ‘3A’), implemented in the municipality of Lo Prado, city of Santiago.</jats:p> </jats:sec> <jats:sec id="sec002"> <jats:title>Methods</jats:title> <jats:p>We carried out a qualitative study with a descriptive-interpretative approach in three schools. We conducted in-depth interviews with students, teachers, health professionals, and school principals (N = 44); and focus groups with students (N = 3), teachers and health personnel (N = 3). The interviews were analyzed using thematic analysis.</jats:p> </jats:sec> <jats:sec id="sec003"> <jats:title>Results</jats:title> <jats:p>Participants highlight the integrative approach to health and to sexual and reproductive health promoted in the 3A Program, which is enhanced by the collaboration of interdisciplinary health teams. Permanent and expedited student access to sexual and reproductive health care is achieved, and affectional bonds are developed between students and the Program’s health staff. The Program assists female participants to imagine and form identities that are not inherently tied to motherhood. It also assists boys and LGBTQ+ adolescents in feeling included as relevant actors in sexual and reproductive health and decision making. The delivery of contraception in schools is highly valued. The most significant challenge identified is ensuring effective and ongoing collaboration between health staff and teachers.</jats:p> </jats:sec> <jats:sec id="sec004"> <jats:title>Conclusion</jats:title> <jats:p>Participants value the effectivity and sexuality component of the 3A Program as an initiative to improve adolescents’ access to sexual and reproductive health care. Our findings suggest that this Program could be replicated throughout the region and the country to improve the quality and accessibility of health services for adolescents.</jats:p> </jats:sec>

    Conocimiento, acceso y uso del sistema de salud en adolescentes migrantes en Chile: resultados de un estudio exploratorio

    <p><bold>Objetivo.</bold> Identificar el conocimiento, acceso y uso del sistema de salud por parte de adolescentes migrantes, y las barreras para el acceso y uso del sistema de salud en dicha población en Chile.</p> <p><bold>Métodos.</bold> Estudio exploratorio observacional, de corte transversal, aplicado en jornada participativa con adolescentes migrantes (N=30) de tres comunas de la ciudad de Santiago, Chile. Estudio multimétodos. Se aplicó una encuesta basal sobre el proceso migratorio y las experiencias en el sistema de salud, con análisis descriptivo de datos. Desde lo cualitativo, se realizaron grupos de discusión (n=5) y talleres participativos (n=2) con adolescentes migrantes.</p> <p><bold>Resultados.</bold> El 60% de adolescentes afirmó desconocer si está inscrito en el sistema de salud, y la mitad no lo ha utilizado. Se identificó que el tiempo de residencia es un factor relevante para el uso efectivo de algunas prestaciones de salud. Se reconocieron barreras administrativas para acceder al sistema de salud, como situación migratoria, percepción de obligatoriedad de compañía de adultos, y experiencias de discriminación en la atención. Se identificaron necesidades específicas en salud sexual y reproductiva, como acceso a preservativos, y salud mental en relación a la atención de especialistas. Como facilitadores del acceso y uso se destacaron las experiencias de buen trato, y la presencia del sector salud en las escuelas.</p> <p><bold>Conclusiones.</bold> Se identificó desconocimiento del sistema de salud, y diversas barreras de acceso y uso de este por parte de adolescentes migrantes. En base a los resultados se proponen recomendaciones generales para el país y la región referidas a la promoción de la salud de este grupo.</p>
